Azelaic acid for redness: what it can (and can't) do

Short answer: azelaic acid is one of the gentler, better-studied options for redness and rosacea-type bumps. Prescription 15% gel is FDA-approved for the bumps and pimples of mild to moderate rosacea [1], and a 2023 systematic review found it improved redness versus a placebo base [2]. Over-the-counter products are usually 10%, which is less studied. Expect some tingling in the first weeks.

How to start

  1. After cleansing, apply a thin layer to the red or affected areas, once a day at first.
  2. Moisturize on top.
  3. Mild burning or stinging is common in the first few weeks and usually settles; stop if it's severe or doesn't settle [1].
  4. Work up to twice a day if your skin is comfortable, and wear SPF 30+ every morning.
  5. Give it 8–12 weeks before deciding.

Pregnancy

ACOG lists azelaic acid among over-the-counter acne ingredients that can be used during pregnancy [4], which is why our quiz uses it as the acne pick for anyone pregnant, breastfeeding or trying to conceive. Some products combine it with other actives that have less pregnancy data, so check with your OB.

When redness needs a dermatologist

Persistent central-face redness, flushing with visible blood vessels, burning, or eye irritation can be rosacea, which a dermatologist can confirm and treat with prescription options. Rosacea bumps aren't acne, so harsh acne products (benzoyl peroxide, strong acids, scrubs) often make them worse.
