Which should you choose?

  • Mostly red, inflamed spots: start with BPO, ideally a 2.5–4% wash or a 2.5% leave-on.
  • Mostly blackheads and bumps on oily skin: salicylic acid is a reasonable start, though adapalene has stronger evidence for clogged pores. See how to use adapalene.
  • Sensitive skin or rosacea: neither is a great fit. Rosacea bumps are not acne. Azelaic acid is gentler, and a dermatologist can confirm what you have.
  • Deep, painful or scarring breakouts: see a dermatologist rather than self-treating.

How to use benzoyl peroxide

  1. Wash form: massage onto damp skin for 1–2 minutes, then rinse. Short contact still reduces bacteria and is easier on skin [4].
  2. Leave-on form: a thin layer over the acne-prone area, every other day at first, then daily if your skin tolerates it.
  3. Follow with moisturizer. Use white towels and pillowcases.

How to use salicylic acid

  1. As a cleanser, or as a leave-on a few nights a week after cleansing.
  2. Don't use it on the same night as a retinoid or another acid while your skin adjusts.
  3. Moisturize after. Stop or cut back if your skin stings or peels.

Can you use both?

Yes, carefully. Many people use a BPO wash in the morning and salicylic acid a few evenings a week. The AAD recommends combining treatments that work in different ways when one isn't enough, but introduce them one at a time so you can tell what's causing any irritation [1][5]. Don't layer BPO at the same moment as prescription tretinoin.
